Health History

A health history is required upon enrollment either online or in person and periodically needs updating for the school nurses to be knowledgeable of chronic health conditions, medication, or nursing procedures needed while at school and for evaluation of illness and appropriate interventions.

Please keep your child at home if:

• If your child has a fever of 100.4 degrees or greater.

• Until your child is fever free without taking fever reducing medications (acetaminophen or ibuprofen) for at least 24 hours.

• If you think your child has chicken pox or other communicable disease.  

• If your child is experiencing diarrhea and/or vomiting (keep home for 24 hours).  

• If your child has "heavy" nasal congestion or a productive, persistent or hacking cough, with or without a fever.

• If any unexplained rash accompanied by a fever is present.   

• Your child has red eye(s), with goopy or crusty drainage coming from their eyes, with or without itching.

Situations where a child may attend school

• Rash - A rash is present, but there is no fever.  Contact your school nurse.

• Common Cold - Your child has symptoms of a mild cold but otherwise feels well. 

• Twenty-four (24) hours have passed after starting an antibiotic for a contagious disease such as strep or pink eye.

Any questions regarding required immunizations for your student should be directed to your health care provider, the Coconino County Public Health Services District (928) 679-7222 or  your child's school nurse. Students will not be allowed to begin school without up to date immunizations with the exception of students with an exemption on file or students covered by the McKinney-Vento Law. An immunization record signed by the health care provider must be on file with the school before a student may enroll.

Medications at school

Prescription medication(s) and over-the-counter (OTC) medication(s) that need to be taken during school hours will be administered by the school nurse or other designated school staff.  A parent/guardian’s signed consent is required for the administration of prescriptions and OTC medications at school.  Both prescription and OTC medications must be given to the school nurse or administrator in the original container with a proper pharmacy label and a “request of medication at school” form must be completed prior to being able to administer the medication(s).

Hearing & Vision Screenings 

Hearing and Vision screenings are generally initiated at the beginning of the school year in conjunction with the Arizona State Law and Department of Health Guidelines. Screenings performed in the health office are not equivalent to a comprehensive hearing/ear or vision/eye exam. 

Hearing screenings performed each year:

• Preschool, Kindergarten, 1st, 3rd, 5th, 7th, and 9th grades.
• A student who is new to the school who does not have documentation of a screening within the last 12 months. 
• Special Education students.
• At the teacher's request. 
• If a student did not pass hearing rescreening the previous school year. 
• Or if the student is repeating a grade. 

Vision screenings performed each year:

• Preschool or Kindergarten (Kindergarten if the student did not attend preschool), 3rd, and 7th grades.
• A student who is new to the school who does not have documentation of a screening within the last 12 months. 
• Special education students.
• A teacher has requested a screening and the student has not been screened in the previous year. 
• The student is not reading at the proficient level by third grade. 

A parent may choose to opt out of hearing and vision screening by completing the opt out form. Please contact your childs school if you would like a a Vision & Hearing opt out form.  

If your child passes hearing and vision, you will not receive notification. 

If your child fails to hear one or more tones during the hearing screening on the first attempt, a rescreen will take place within 10-30 days. If your student fails the second screening, a referral letter will be sent home. For both hearing and vision, if a referral is sent home, the expectation is that a professional evaluation will be completed. If you need assistance with finding a provider, please contact your health office, nurse or call your child’s doctor. 

Early identification and treatment of vision or hearing issues can be key to preventing long-term complications.
